We would like to thank you for considering us to be the ones to help you share your captured moments. We want to get to know you little, so we may better serve you and help you share your message. Taking a few minutes to fill out this questionnaire will give us an idea of who you are, and what you want to share. We will also be more tuned-into your needs and expectations moving forward.
Fill out as much as you would like. The more we know about who you are, the better we can portray that in your images.

We want to make this experience as awesome as we can. If there is someone that would like to come and observe, there aren't any limits.
